Only what you type into the contact form.
A name, an email address, a phone number, and a reason for reaching out. Nothing else is asked, measured, or inferred.
No diagnoses, no health histories, no protected health information.
The form has no field that could hold them. Anything clinical belongs to a conversation inside the practice, under its own consent, never to a web page.

No analytics, no cookies, no third-party requests of any kind. Every font, image, and script is self-hosted. Open your browser’s network panel and count the origins: there is one.
Read by a physician. Held quietly.
Never sold, never shared, never used to market to you. If you ask for it to be deleted, it is deleted.
The form is not yet connected.
When the practice opens, submissions will travel through an approved secure endpoint. Until then the form sends nothing and stores nothing, and it tells you so, plainly, at the moment you press its button.
